| Description: | OakView is currently seeking an RN Supervisor who will provide direct therapeutic, restorative and preventive care to residents with identified health needs. Responsibility for care extends from admission through discharge within structured health care settings and can extend into the unstructured health care settings of residential living. Essential duties and responsibilities: Provides leadership for the team through delegation of assigned duties and monitors all resident care and activities on the unit; Coordinates monthly work schedule and daily assignments to ensure the delivery of cost effective, quality resident care; Monitors and ensures completion of all medical record documentation, audits and infection control log; Ensures consistency of resident care through shift to shift rounds, every two hour rounds, change of condition report, and follow-up on doctor and family calls; Participates on the interdisciplinary team by ensuring that resident assessments and care plans are completed, reviewed and updated in a timely manner, and by attending scheduled care plan conferences; Monitors the Restorative Nursing Program under the direction of the Director of Nursing and the registered therapists; Admits, transfers, and discharges residents in coordination with other discipline; Interacts with discharge planners; Requests needed information from hospitals and responsible parties for admissions; Coordinates nursing services with facility and community resources; Orders and administers medications, performs treatments, schedules tests and other diagnostic work as ordered by physicians; Assures that residents receive the prescribed diets; Reports resident variations and implement nutritional interventions as appropriate; Documents weekly assessments focusing on care plan goal achievements following facility schedule; Reviews weekly assessments for completeness and accuracy; Initiates appropriate revisions. Documents significant occurrences in a timely manner; Demonstrates working knowledge of universal precautions and other infection control procedures; Documents all pressure sores, skin tears, and infections in Infection Control Surveillance Book; Supervises staff compliance; Demonstrates and demands required infection control procedures; Report breaks in technique to Director of Staff Development; Demonstrates ability to assist residents in achieving and maintaining highest level of independence; Supervises staff and encourages resident independence and autonomy. Qualifications: Current licensure by the California Board of Registered Nursing; More than two years experience as a registered nurse in LTC with prior charge nurse or supervisory experience preferred; BSN preferred with documented seminars/course work related to management, supervision, advanced clinical skills, geriatrics, rehabilitation, Alzheimer?s management; Computer literate. |
